font-family:Calibri">As we have discussed, the Champaign-Urbana Public Health District (CUPHD) is conducting the next cycle of the Illinois Project for Local Assessment of Needs
(IPLAN) in collaboration with <st1:PlaceName w:st="on">Carle</st1:PlaceName> <st1:PlaceName w:st="on">
Foundation</st1:PlaceName> <st1:PlaceType w:st="on">Hospital</st1:PlaceType>, <st1:place w:st="on">
<st1:PlaceName w:st="on">Presence</st1:PlaceName> <st1:PlaceName w:st="on">Covenant</st1:PlaceName>
<st1:PlaceType w:st="on">Hospital</st1:PlaceType></st1:place>, and the <st1:Street w:st="on">
<st1:address w:st="on">United Way</st1:address></st1:Street>. We partner in this process every three years. A survey tool has been developed to assess
<st1:place w:st="on"><st1:PlaceName w:st="on">Champaign</st1:PlaceName> <st1:PlaceType w:st="on">
County</st1:PlaceType></st1:place> resident's perception of health and wellness in the communities where they live. survey should not take more than 5-7 minutes to complete. The surveys will be used to inform the development of plans to improve the quality of life in our county. The results of this IPLAN process will be available later this year and will be accessible
