<st1:City w:st="on"><st1:place w:st="on">Champaign</st1:place></st1:City> in July 2015 for new group leaders for the
<i><u><span style="font-style:italic">Diabetes Self-Management Program (DSMP)</span></u></i> . This is an evidence-based program that was developed by Kate Lorig out of
<st1:place w:st="on"><st1:PlaceName w:st="on">Stanford</st1:PlaceName> <st1:PlaceType w:st="on">
University</st1:PlaceType></st1:place> . This program is the diabetes specific version of the
<i><span style="font-style:italic">Chronic Disease Self-Management Program (CDSMP)</span></i>, known in IL as
<i><span style="font-style:italic">Take Charge of Your Health: Live Well, Be Well</span></i>, which was also developed by Kate Lorig out of
<st1:place w:st="on"><st1:PlaceName w:st="on">Stanford</st1:PlaceName> <st1:PlaceType w:st="on">
University</st1:PlaceType></st1:place> and has been offered in IL over the past 8+ years.

<st1:Street w:st="on"><st1:address w:st="on">405 S. State St.</st1:address></st1:Street> ) on the
<b><u><span style="font-weight:bold">2 Thursdays and Fridays July 23-24 and July 30-31, 2015</span></u></b> . The 4 training days will be scheduled from 9:00 - 4:00 each day. This will provide the required training for individuals who are interested in serving
as co-facilitators for the 6 week workshop. </span></font><font size="4" face="Times New Roman"><span style="font-size:13.0pt;font-family:"Times New Roman""><o:p></o:p></span></font></p>

<b><span style="font-weight:bold">Those within PSA 05 will not be charged a fee</span></b>. Each person will be responsible for his/her own lunch; however, we will be ordering lunch each day of the training for any who would like to pay $10 per lunch. We are
pleased to be able to offer CEU's for any participant who requests them at the cost of $10.
