Good Afternoon, Everyone!

Please see the attached flyer and brochure explaining the Chronic Disease Self-Management evidence-based program. Family Service is excited to begin offering this class in person again! The next program series will be held Tuesday afternoons at the Champaign Public Library from 1:30 - 4:00pm, May 3rd - June 7th. This program is open to the community and free of charge. Please register by Thursday, August 28th by calling Stacie at the Family Service Senior Resource Center at 217-352-5100.

As someone that has gone through the course and has been trained as a leader, I can attest to the valuable techniques and information I learned in the program that I still use today. Please share this flyer with anyone that may be interested in joining the upcoming classes!
