Hi Everyone,

See below and attached for information about Carle providing in-home COVID vaccinations. Please note that you must be a current Carle patient and you must meet the CMS homebound definition in order to qualify. Thanks!


Sample note for your listserve:

Carle Home Health Services in partnership with the Champaign-Urbana Public Health District (CUPHD), is now vaccinating homebound patients.

Who: Carle patients who live in Champaign County and are homebound
To schedule: Call (217) 902-6100 or request an appointment online at https://tinyurl.com/CChomebound.
Please note: Carle will only offer this service to current patients who are 18 and older.

Homebound persons include those who need the help of another person or medical equipment such as crutches, a walker, or a wheelchair to leave their home, or their medical provider believes their health or illness could get worse if they leave their home, and they typically do not leave their home.

Homebound patients not currently seen at Carle should call (217) 531-5369 for direction.
