Hello All,

 Family Service is offering a training opportunity for those interested in offering the Chronic Disease Self-Management Program (CDSMP) to seniors, their caregivers, and others.  The program is appropriate for those with diabetes, arthritis, heart conditions, or any other chronic health issue.

The class is being offered free of charge.  Those interested in being leaders do not have to be social service professionals.  The class is scripted and easy to follow for those who are comfortable talking with groups. The Leaders’ Training will be held at Family Service at 405 S. State Street in Champaign on August 5, 6, 12, and 13 from 9:00 to 5:00 each day.  Participants must complete all 4 days to be certified and must agree to hold their first class within 6 months of being trained.

Leaders will hold classes in their community for 8-12 people.  Classes are highly participatory. Mutual support and success builds participants’ confidence in their ability to manage their health and maintain active and fulfilling lives.  They meet for 2 1/2 hours per week, once a week for 6 weeks.

Physicians and other health professionals both at Stanford and in the community have reviewed all materials in the workshop which address a broad range of topics including:

·       Techniques to deal with problems such as frustration, fatigue, pain and isolation.

·       Appropriate exercise for maintaining and improving strength, flexibility, and endurance

·       Appropriate use of medications

·       Communicating effectively with family, friends, and health professionals

·       Nutrition

·       Decision making

·       Evaluating new treatments

 Studies show that those who took this evidence-based Program, when compared to those who did not, demonstrated significant improvements in exercise, cognitive symptom management, communication with physicians, self-reported general health, health distress, fatigue, disability, and social/role activities limitations. They also spent fewer days in the hospital, and there was also a trend toward fewer outpatient visits and hospitalizations.

Each person will be responsible for his/her own lunch; however, we will be ordering lunch each day of the training for any who would like to pay $10 per lunch. We are pleased to be able to offer CEU's for any participant who requests them at the cost of $15.
