Dear All:

Here is a link to a survey for use by people who have mental health concerns, substance use disorders, or intellectual/developmental disabilities and for anyone with an interest:

The more people we hear from, the better. Your support is vital in helping us reach those most affected by the local service systems, so I hope you will share this link with people you serve.

We also have paper copies of the survey which may be easier for many. Please let me know if you would like some, and I will send with a self-addressed return envelope. If interest in the survey will be stronger by us visiting a group of people you serve, please let me know that as well!

We will close the survey on Friday, August 20.
