Regarding our discussion about advanced directives, I have provided a link to the Planning Ahead <https://www.healthalliance.org/media/Resources/IL-adv-directive-guide.pdf>  document I shared yesterday. It includes some printable forms that might be useful to the group. I have also attached a link to our Health Alliance website which offers a number of resources for people who are wondering How do I plan for the Future<https://www.healthalliance.org/Content/compassionate-care>.

We also offer a number of Power Point presentations (attached) as part of our Community Education Mission. One of the most popular is The ABCs of Medicare. If anyone would like one of our Community Liaisons to present an  educational program just let me know. We would be happy to set up a time for their group.
