My friend Candi Crause of C-U Public Health is distributing a valuable community health survey and is very interested in insuring that older adults are included.  If you are able to distribute the link to the survey, it would be much appreciated!

As we have discussed, the Champaign-Urbana Public Health District (CUPHD) is conducting the next cycle of the Illinois Project for Local Assessment of Needs (IPLAN) in collaboration with Carle Foundation Hospital, Presence Covenant Hospital, and the United Way.  We partner in this process every three years.  A survey tool has been developed to assess Champaign County resident's perception of health and wellness in the communities where they live.  The survey can be accessed electronically through (https://www.surveymonkey.com/r/IPLAN17). ​​The survey should not take more than 5-7 minutes to complete.  The surveys will be used to inform the development of plans to improve the quality of life in our county.  The results of this IPLAN process will be available later this year and will be accessible on the CUPHD website (www.c-uphd.org<http://www.c-uphd.org/>).
