This is a friendly reminder about the upcoming CDSMP Leader Training Program February 2/3 and 9/10 at Illinois Valley Community College (IVCC). I would love to get some more registration forms from folks who'd like to attend.
If you are considering getting trained for CDSMP, please make sure you 1) either have a co-leader in your area to facilitate the program with (it's a co-led program), or 2) identify a person to take this 4-day training with you.

Hello All,

Attached is registration information for the Stanford Chronic Disease Self-Management Program Leader (aka Facilitator) Training program to be held on the campus of IL Valley Community College Thurs/Friday Feb 2/3 and 9/10. In order to participate in this training, you must be able to commit to all 4 days. If you have extenuating circumstances, please contact me individually. The registration deadline is Monday, January 23rd. This training is likely to fill fast (we cap at 20) so registration is prioritized for 1) Extension staff and partners first, and 2) grant partners, and 3) first come-first served after that.

Master Trainers for this certification program are Chelsey Byers and Heather Coyle (both with U of IL Extension.
If you are a partner via Extension or a delivery partner for the statewide Administration on Community Living (ACL) Chronic Disease Self-Management Education (CDSME) grant program, this training will be FREE OF CHARGE for you. **Note: This program is team-taught so it is ideal to train in partners unless you already have a certified facilitator you can lead the program with in your area.

If you plan to stay overnight, please let us know by indicating "YES" on the reg. form and we can arrange a room block at a nearby LaSalle/Peru Hotel and make that available. Directions and a campus map are also attached.
