Hello All,

I am pleased to share with you that a training will be offered in Champaign in September for new group leaders for the Take Charge of Your Health: Live Well, Be Well workshop. This is the official name that has been adopted throughout Illinois for the evidence-based Chronic Disease Self-Management Program (CDSMP), which was developed by Kate Lorig out of Stanford University.  We have not offered this training for a few years in Champaign County so we are hopeful that these dates will work for the interested participants.

Rosanna McLain and I will be offering the training in Champaign at Family Service ( 405 S. State St. ) on the 2 Thursdays and Fridays September  15-16 & 22-23, 2016 . The 4 training days will be scheduled from 9:00 - 4:00 each day. This will provide the required training for individuals who are interested in serving as co-facilitators for the 6 week workshop.
For individuals living outside PSA 05, the cost is $175 for the training. Those within PSA 05 will not be charged a fee. Each person will be responsible for his/her own lunch; however, we will be ordering lunch each day of the training for any who would like to pay $10 per lunch. We are pleased to be able to offer CEU's for any participant who requests them at the cost of $15.
Some of you in this email are currently serving as leaders. Others may have expressed interest in the past and have been supporters of these evidence-based programs. I am sending this information in hopes that you will consider participating in the training and/or sharing the information with individuals who might be strong leaders for this valuable program. Please forward the information to any individuals you think might appreciate knowing about the training and let interested individuals know that Rosanna and I are very happy to discuss the program and the training specifics with anyone who is interested.

I have attached a registration form for your review and for sharing. Please contact her soon if you are interested in participating in the program so we can reserve you a spot, as the group size will be limited. We will then send further information to the participants as the time grows nearer.
