Subject: Community Wide Health Screening

The National Kidney Foundation of Illinois is partnering with Presence Center for Healthy Living to provide a free community wide comprehensive health screening.  Please pass this along to anyone you think would benefit (don't forget to include yourself!)  I have attached a flyer with more details.  If for some reason you cannot open the attachment, some details are listed below.   In addition to the Kidney & Diabetes screenings, we will have several other health and wellness screenings, information, and resources on site.

If your organization has a van that would be available to provide transportation for homebound seniors please contact Bevin.Page at presencehealth.org<mailto:Bevin.Page at presencehealth.org>   Bevin would love to be able to offer this to seniors who are not able to drive through the Faith in Action program!

Time:             12:00 pm - 4:00 pm

No appointment necessary, must be 18 years or older

National Kidney Foundation screenings include:  blood pressure, blood sugar (fasting not required), body mass index (BMI), waist circumference, urinalysis (urine sample will be required), blood draw (if necessary), talk about your results with a nurse

Additional screenings, information and resources:  hearing, lung function (spirometry), vascular (ABI), nutrition, social wellness, Medicare information, Community Resource Center, spiritual room, and hand massages.
